The foundation for this work was completed in 2023 with the development of the Well-Being Collaborative, Steering Commitee and changes in staff job descriptions. The staff convene and facilitate monthly meetings to bring awareness of community gaps, resources and emerging needs. The coalitions are Salud para Todos, Housing and Emergency Services Committee, Food Leaders, Violence Prevention and Substance Abuse prevention. As emerging work emerges from these workgroups, it is shared with the Well-Being Collaborative Steering Committee to help prioritize the work or bring in new partners to lead the work. A community dashboard holds this information. We also have a new community process online, www.bcchp.org/innovative, to support the Steering Committee work, see the matrix to help define our process. Several activities will take place in 2025 in this area for food security, violence prevention, substance abuse, mental health training and resources, and housing. These activities are too numerous to share, apx 33 different prevention strategies are currently funded. Accessing food from local public or faith-based pantries may be a barrier for some who lack transportation. Hours of operation for pantries may also be a barrier for some to access food. We are working with our numerous partners in distributing food purchased with Local Food Purchase Assistance grant funds to locations in the four-county area which include communities where food pantries do and do not exist. Partners are helping partners transport food to places in our four-county area where pantries don't exist and where there is need based on food insecurity data and by speaking with local community members as to what needs exist.

In conjunction with CFP's fiscal lead, Columbus Area United Way, we've engaged partners and the NE Dept of Transportation on the topic of a potential transit system between Columbus & Schuyler along Highway 30 specifically for employee transport. Similar to other communities in the state, transportation is a barrier for many of CFP's families. During this nearly two-year process of regular monthly meetings with partners & NDOT, we've conducted a successful transportation survey with over 900 responses from community members which reflects the need for a transit system. City/county funds have been secured with additional funds needed to meet 25% of the overall annual transit budget. CAUW provided a presentation to business CEOs on survey results & benefit to them as employers for a system to transport employees to & from work. We are gathering stories from those with lived experience and the impact of transportation on their jobs/family, etc. as well as letters from partner to further show impact transit has on employment and overall health of community members. Stories and letters will be used for educational purposes as we advocate for the cause.

Through a grant with Nebraska Department of Economic Development, CIN is focusing on a specific geographic area of Hastings to combine a poverty alleviation initiative (Bridging Forward) with talent acquisition, retention and development for businesses in the area and for people who live or work in the Qualified Census Tract (QCT). The goal of the initiative is to support 80 individuals to achieve 200% of the Federal Poverty Line by the end of 2026 and to support 30 individuals to overcome Benefit Cliff barriers to employment or advancement

Bi-weekly case mangers across our community who participate in community response gather for on going training such as Families Thrive, Bridges out of poverty, case staffing, FindHelp training along with support to each other. The case managers come up with trainings they need to be successful in their role. This group also leads committee work that comes out of the community impact meetings. Family Coaching The family coach works directly with families to identify and address barriers to accessing basic needs such as housing, food, transportation, and healthcare. By offering personalized support and guidance, the coach helps families navigate complex systems and connect with local resources, ensuring they can meet their immediate needs and build long-term stability. Central Navigation Central Navigation provides a streamlined, easy-to-access point for families to find services and support. It helps families quickly connect with available resources, reducing the time and effort spent searching for help. Central Navigation also assists with referrals to services, including those for youth, ensuring a holistic approach to meeting family needs. Connecting Families with Community Resources Both family coaching and Central Navigation collaborate with local service providers, ensuring that families have access to food pantries, housing assistance, healthcare services, childcare, and more. By removing administrative barriers and offering hands-on support, these programs improve access to critical services for youth and families in the community.

The T & I classes built the pantries and the collaborative initially stocked them and spread the word/promoted community awareness of the take what you need and give what you can pantries. We currently have pantries located in Atkinson, Butte, Chambers and O'Neill. Stuart is in the process of building a pantry for their community.

As we continue to collect data around the Emergency Rental Assistance Program we have identified that households are not achieving sustainability like we had hoped. We believe this is attributed to the lack of accountability they were required to have in achieving short-term goals through their housing assessments and the lack of coaching that was available to help them be accountable. As families continue to reach out for housing supports (food, utilities, rent, etc) we will work toward a system that identifies one-time need versus long term or chronic needs. By doing so we will be able to determine the need for coaching and what that model will provide to the client to help in reaching sustainability.

The goal of this project is to enhance and provide Central Navigation services through a resource center hotline, NE 2-1-1. The goal is to give our community members a one-stop shop for resource referrals and to answer questions. The Central Navigator located at 2-1-1 will receive the call. The caller/client will identify the need. The CN will gather information to complete intake and then will refer to potential community resources. If resources are unable to assist, the CN will refer the client to ACT for direct intervention. The goal is to enhance this project to provide a central calling system that provides that "warm" hand-off approach with our community members in need. We will gather data on elderly, single adults, young people, children and families.
