Details
The Buffalo County Well-Being Collaborative meets quarterly and refreshes the membership of the six key components of the collaborative. This body of work can be found at www.bcchp.org/2030vision. The Collaborative revisits the foundation at each meeting, promotes the elements through reinforcing language throughout all communication and offered training for new volunteers. The foundation of the collaborative is built on collective impact, results-based accountability, wellness indicators (data), community gaps, infrastructure and a shared vision.

Building on Strategy STRG - 1053, outcomes from the 33 prevention strategies are lifted up to the steering committee and reported in the annual year end Community Well-Being Report of NCFF. A report can be shared by contacting us at info@bcchp.org.

Our community lifts up 22 Community Work Groups (Committees, Coalitions, Task Forces, etc.) who meet monthly as well as five community resource gatherings from seniors to persons with disabilities. Our website currently defines the Community Work Groups by purpose and goals, www.bcchp.org/communitywork. In 2025, we hope to add in the five community resource groups in which people can attend and learn about current workforce resources, senior services, persons with disability services and more. These groups engage many Community Support Specialists, to help guide supports, learn or share resources. This informal gathering of people is working for our community; however, more conversations are needed to continue to build what works best for the community in which they live.

In collaboration with the City Councils and school districts we will work to implement a Community Cafe(s) in each city within our county. Each cafe will work toward supporting self advocacy, identifying the service gaps within their community, connect home school providers/parents, and create the idea of getting to know your neighbor.
